Whether you’re out for a meeting, appointment, professional development, or sick day, it’s always helpful to know how the day went while you were away. Feedback from a substitute teacher can provide valuable insight into student behavior, completed work, and anything important that may have happened during the day.
That’s why I created this free substitute teacher feedback form! It’s a simple and easy way for substitute teachers to communicate important information before they leave for the day. The form allows substitutes to leave notes about completed lessons, student behavior, classroom concerns, and anything else you may need to know when you return.
I like to leave this form right on top of my substitute plans with a quick note asking the substitute teacher to complete it before leaving.
